We will collect a comprehensive medical and family history. This is the cornerstone of the PPE and will focus on the athlete's abilities for their sport participation. The American Heart Association recommendations pre-participation cardiovascular screening for competitive athletes. The medical and family history provided by the athlete and the parents or guardians should always be reviewed carefully. Both parties should be questioned and specific answers confirmed to verify the most accurate information. Here are areas that will be evaluated:
PERSONAL HISTORY
- Exertional chest pain/discomfort
- Unexplained syncope/near syncopec
- Excessive exertional and unexplained dyspnea/fatigue, associated with exercise
- Prior recognition of a heart murmur
- Elevated systemic blood pressure
FAMILY HISTORY
- Premature death (sudden and unexpected, or otherwise) before age 50 y due to heart disease, in >1 relative\
- Disability from heart disease in a close relative age <50 y
- Specific knowledge of certain cardiac conditions in family members: hypertrophic or dilated cardiomyopathy, long-QT syndrome or other ion channelopathies, Marfan syndrome, or clinically important arrhythmias

Pre-participation Physical Examinations and Disqualifying Conditions
