WHAT TO BRING TO YOUR APPOINTMENT

 

Complete all appropriate forms for the physician you will be seeing before your visit, if possible.

 

Please bring the following items to every visit:

• Driver’s License/Other Valid Government-Issued Photo ID
• Insurance Card and any co-payment due for the visit
• Referral forms, if applicable
• X-rays and any related studies, if applicable
• Any necessary new patient or follow-up forms

HOW TO PREPARE FOR YOUR PHYSICIAN'S QUESTIONS

 

Help your physician diagnose and treat your condition by being prepared to answer the following questions:

• What are your main symptoms?
• How long have you had your symptoms?
• What were you doing when your symptoms started?
• Have you had this problem in the past?
• What was the diagnosis?
• How was it treated?
• How and when did an injury occur? How was it treated?
• Have you had any injuries in the past to the same area? Do you have any continuing problems because of the previous injury?
• Have you ever had surgery in that area?
• What activities, related to sports, work, or your lifestyle, make your symptoms better or worse?
• Do you think that activities related to your job or hobbies caused your symptoms?
• What home treatment measures have you tried? Did they help?
• What nonprescription medicines have you taken? Did they help?
• Make a list to avoid forgetting any specifics.

 

 

LATE ARRIVAL, CANCELLATION & NO-SHOW POLICIES

 

Please arrive for your scheduled appointment on time. We recommend that new patients arrive 20 minutes early to fill out any necessary paperwork. Patients who arrive more than 15 minutes after their scheduled time may need to be rescheduled.

 

All of our physicians and therapist at Mississippi Sports Medicine believe strongly in the importance of every patient keeping their appointments. When patients fail to keep their scheduled appointment, they miss out on crucial medical information regarding their recovery. Unfortunately, our clinic has experienced an increased number of No-Show appointments. Therefore, we have had to institute a policy of billing patients for their missed appointments.

 

• Please be advised that we require no less than a 24-hour notice whenever an office/physical therapy appointment is cancelled, and 48 hours for all surgery appointments.
• If you have an appointment on a Monday before 12:00 pm, you must call before 12:00 pm on Friday before your scheduled appointment of you will be billed a No-Show fee.
• Insurance plans are not responsible for No-Show Fee’s: it is the responsibility of the patient to pay the bill upon receipt.
• If you need to cancel an appointment and it is after hours or during the weekend, you may leave a message with our answering service. We recommend that you make a note of the name of the service operator/receptionist whenever you call to cancel or reschedule an appointment.

 

FEE SCHEDULE FOR NO SHOWS & CANCELLATIONS

 

• Physician Office Visit: $30.00 must be canceled within 24 hours.
• Physical Therapy Visit: $30.00 must be canceled within 24 hours.
• Surgery $300.00 must be canceled within 48 hours.

**If a patient is a no show 3 times, they may be discharged from our practice**

 

HOW TO PREPARE FOR X-RAYs AND MRIs

 

To avoid delays for all patients undergoing an X-ray or MRI, the department aims to reduce the number of patients who need to undress for their X-ray. RENEWING YOUR PRESCRIPTION

 

Please call your pharmacy to renew your prescription. They will contact your physician for any necessary information. Please allow at least 24 to 48 hours to accommodate your request, as each request has to be approved by a physician and all physicians are not in the clinic every day.

 

If your prescription is running low, please do not wait until the weekend or until your medication has completely run out. If you are planning to travel out of town, please make sure you have enough medication to take on your trip.

Need your FMLA or Disability Forms completed?

1.   Scan & Submit Request (Start)
      You may submit blank forms and authorization in-person or via Request Manager at https://requestmanager.healthmark-group.com
2.   HealthMark Completes Forms (2-5 days)
      HealthMark will complete the form. As forms are being processed, please look for email correspondence from the HealthMark Team for further updates.
3.   Invoice and Forms Ready (1 day)
      The invoice will be released via email with instructions on making payment.
4.   Make payment and Access Forms (Due for Release)
      Once payment is complete, forms will be available through Request Manager.

      *The total process time of requests is approximately seven days.

• Multiple forms submitted together will cap at an $80 fee.
• You will be notified of all fees prior to the release of forms, and forms are released only after payment.

State Forms

• Submit State forms on the day of surgery or any time after surgery. Forms will be completed and made available within 24 hours.\
• All state forms are free of charge. You will not receive an invoice.

• Initial statuses are available 48 hours after submission.

What can you do to check the status of form(s)?

• You may contact HealthMark’s Requester Support team at 800-659-4035.
• You may email our FMLA/Forms team directly for status at fmla@healthmark-group.com. HealthMark will also send updates on status via email.

What notifications will you receive?

HealthMark will make their first contact with you via text message to inform you that your FMLA/STD form has been received and that future correspondence will take place via email. Within 24 hours you will receive an email from the forms team. If you have not received your email in 24 hours, please call Requester Support at 800-659-4035 or email the team directly at fmla@healthmark-group.com. (Please check your inbox and spam/junk folders as emails have been known to get trapped there)
